UEN: 202339132R
Please read this document carefully before your first session. By proceeding with services, you acknowledge that you have read, understood, and agree to the terms below.
1. Nature of psychological services
Psychological services at Lightfull Psychology may include clinical assessment, psychological testing, psychotherapy (including Cognitive Behavioral Therapy, Acceptance and Commitment Therapy, and other evidence-based approaches), and consultation. These services are provided by Dr. Ong Mian Li, a clinical psychologist registered on the Singapore Register of Psychologists (SRP No. 2512014).
Psychology is not an exact science, and no guarantees can be made regarding the outcome of services. However, all services are delivered in accordance with the ethical guidelines of the Singapore Psychological Society (SPS) and the American Psychological Association (APA).
2. Confidentiality
All information shared during sessions is treated as strictly confidential. Information will not be disclosed to any third party without your written consent, except in the following circumstances as required or permitted by law:
- There is a risk of serious harm to yourself or others
- There is suspected abuse or neglect of a child or vulnerable person
- Disclosure is required by a court order or mandated by Singapore law
- Disclosure is necessary for the purpose of obtaining professional consultation or supervision (in which case, identifying information is removed where possible)
For clients under 18, parents or legal guardians have the right to request general information about treatment progress. However, the specific content of therapy sessions is kept confidential to preserve the therapeutic relationship, unless there is a safety concern.
3. Personal data protection (PDPA)
Lightfull Psychology collects, uses, and stores personal data in accordance with the Personal Data Protection Act 2012 (PDPA) of Singapore. Personal data collected includes your name, contact details, medical and psychological history, and session notes.
Your personal data is collected for the following purposes: providing psychological services, communicating with you about appointments and treatment, billing and payment processing, and complying with legal and professional obligations.
Your data is stored securely and retained for a minimum of 7 years from the date of your last session, in line with professional guidelines. You may request access to or correction of your personal data by contacting the practice directly.
4. Fees and payment
Fees are outlined on the Fees & Payment page. Payment is due at the time of each session unless otherwise arranged. Lightfull Psychology reserves the right to adjust fees with reasonable notice.
5. Cancellation policy
A minimum of 24 hours’ notice is required to cancel or reschedule an appointment. Late cancellations (less than 24 hours) and no-shows will be charged the full session fee. This policy exists to ensure that appointment slots remain available for all clients.
6. Communication
Communication between sessions (via email, text, or messaging apps) is limited to scheduling and administrative matters. Therapeutic advice or clinical guidance will not be provided outside of scheduled sessions. In the event of a mental health emergency, please contact the Institute of Mental Health (IMH) emergency line at 6389 2222 or go to the nearest hospital emergency department.
7. Records and legal requests
Clinical records are maintained in compliance with professional and legal standards. Lightfull Psychology does not provide forensic evaluations, custody assessments, or court testimony. If clinical records are subpoenaed or requested for legal proceedings, an additional fee will apply for the preparation and release of records, billed at SGD 500 per hour. This policy is in place because clinical notes are written for treatment purposes and are not intended for legal use.
8. Termination of services
Either party may terminate the therapeutic relationship at any time. If Dr. Ong determines that the services offered are no longer appropriate for your needs, he will discuss this with you and provide referrals to other professionals where possible.
9. Consent for minors
For clients under the age of 18, consent must be provided by a parent or legal guardian. The consenting adult agrees to the terms of this document on behalf of the minor. Where appropriate, the minor’s assent will also be sought.
